How do I read a row of this table?
Find the substance, then take the code from the column matching the documented intent. The codes are T-code stems; the code page adds the encounter 7th character (A, D, or S) and confirms billability.
Why do some substances have empty columns?
Not every substance has a code for every intent. An empty cell means the classification does not provide one, and the parent entry or the classification entries such as drug NEC apply.
