2026 ICD-10-PCS Alphabetic Index ICD-10-PCS
The Alphabetic Index is the official entry point into ICD-10-PCS. It files 3,344 main terms and 24,789 index entries: procedure names like cholecystectomy and tracheostomy, the root operations themselves, device and brand names, and anatomical terms. Unlike the ICD-10-CM index, most entries do not end at a complete code: they point to a PCS table (the first three or four characters), and the coder completes the remaining characters inside that table. Look up the term, follow it to the table, then build and verify the full seven-character code on its code page. The ICD-10-PCS lookup tool searches the same data by keyword.

What is the ICD-10-PCS Alphabetic Index?
It is the official CMS lookup that maps the terms clinicians document - procedure names, eponyms, root operations, device and brand names, anatomy - to the ICD-10-PCS code tables. Coders start in the Index, follow the entry to a table identified by the first three or four characters, and complete the full seven-character code inside that table. CMS publishes it with every fiscal year release.
Why do most entries point to a table instead of a full code?
ICD-10-PCS codes are built, not picked: every code has seven characters for section, body system, root operation, body part, approach, device, and qualifier. The Index gets the coder to the right table - Cholecystectomy points to 0FB4 (Excision of Gallbladder) and 0FT4 (Resection of Gallbladder) - and the remaining characters are chosen from that table. Only a small set of entries, like Bronchoscopy, resolve to one complete code.
What do "see" and "use" mean in the index?
A "see" reference redirects a documented term to the main term that holds the codes: Cardiorrhaphy says see Repair, Heart and Great Vessels. A "use" reference maps a documented anatomical or brand name to the PCS value that represents it: Cerebral aqueduct says use Cerebral Ventricle, and a stent brand name points to the Intraluminal Device value. Both are followed, never coded from directly.
