Other disorders of iris and ciliary body (H21) ICD-10-CM
The H21 code range covers other disorders of iris and ciliary body with 126 ICD-10-CM diagnosis codes. 97 of them are billable and valid for claim submission in fiscal year 2026, and the category headers group them but cannot themselves be billed.
Type 2 Excludes
A type 2 excludes note represents "Not included here". An excludes2 note indicates that the condition excluded is not part of the condition represented by the code, but a patient may have both conditions at the same time. When an Excludes2 note appears under a code, it is acceptable to use both the code and the excluded code together, when appropriate.

Clinical Terms in This Code Range
Definitions from the National Library of Medicine for conditions coded in the H21 range.
Glaucoma
An ocular disease, occurring in many forms, having as its primary characteristics an unstable or a sustained increase in the intraocular pressure which the eye cannot withstand without damage to its structure or impairment of its function. The consequences of the increased pressure may be manifested in a variety of symptoms, depending upon type and severity, such as excavation of the optic disk, hardness of the eyeball, corneal anesthesia, reduced visual acuity, seeing of colored halos around lights, disturbed dark adaptation, visual field defects, and headaches. (Dictionary of Visual Science, 4th ed)
Hyphema
Bleeding in the anterior chamber of the eye.
Iridocorneal Endothelial Syndrome
A grouping of three closely linked conditions: iris nevus (or Cogan-Reese) syndrome, Chandler Syndrome, and essential (progressive) iris atrophy. The most common features of this syndrome are the movement of endothelial cells off the cornea onto the iris leading to corneal swelling, distortion of the iris, and variable degrees of distortion of the pupil. The abnormal cell movement plugs fluid outflow channels of the eye causing GLAUCOMA.
About the H21 Code Range
The ICD-10 code section H21 covers various other disorders of the iris and ciliary body, key parts of the eye involved in controlling light and focusing. These codes are used to classify specific eye conditions affecting these structures beyond common or well-known diagnoses.
This section includes codes for conditions such as hyphema (H21.0), known by terms like spontaneous hyphema or iris bleeding, which refers to blood in the eye's anterior chamber. It also covers vascular disorders (H21.1X), including rubeosis iridis or neovascular glaucoma, linked to abnormal blood vessel growth from diseases like diabetes. Other notable conditions include various degenerations of the iris and ciliary body (H21.2), pigmentary iris changes such as pigment dispersion syndrome (H21.23), and cysts of the iris or ciliary body with different etiologies (H21.3). Adhesions and disruptions like anterior and posterior synechiae which can lead to angle-closure glaucoma are coded in H21.5x. Uncommon disorders such as floppy iris syndrome (H21.81) are also included. These detailed codes help healthcare providers document the exact eye disorder, specifying affected eyes when necessary, aiding proper diagnosis and treatment planning.
