ICD-10-PCS Code Updates for FY 2027

The FY 2027 ICD-10-PCS update adds 181 new procedure codes, effective October 1, 2026, bringing the inpatient procedure code set to 79,256 codes. Section X New Technology accounts for 38% of the additions. Every changed code is listed on this page, organized by section.

Where the New Codes Landed

ICD-10-PCS growth is concentrated in Section X, where CMS codes procedures that use newly approved devices, substances, and technologies. The traditional sections still pick up targeted additions each year.

The table below shows all 17 sections of the classification and how the FY 2027 additions are distributed across them.

Total Codes is the complete FY 2027 count for each section as loaded in the ICD List database. Net change against FY 2026 can differ from the new-code count when codes are deleted or replaced in the same cycle.

What Changed: the Big Themes

The sections below saw the most movement in the FY 2027 release. Counts and example codes come straight from the official release files.

Section 0: Medical and Surgical

This section received 74 of the 181 new procedure codes in the release, 41% of all additions. New codes include 028F3Z0 (Division of Aortic Valve, Perc Approach, Native Leaflet), 028F3Z1 (Division of Aortic Valve, Perc Approach, Prosthetic Leaflet), and 028G3Z0 (Division of Mitral Valve, Perc Approach, Native Leaflet).

Spine surgery leads the Medical and Surgical additions: 24 fusion codes for custom-made, anatomically designed interbody fusion devices at the thoracolumbar, lumbar, two-or-more lumbar and lumbosacral joints, each with an anterior or posterior approach to the anterior column and open, percutaneous or percutaneous endoscopic access, starting at 0RGA0E0. They replace the 12 Section X codes for the same devices (XRGA0R7 through XRGD4R7), which are deleted. Endoscopic drainage of the gallbladder, bile ducts, pancreatic ducts and pancreas with a drainage device now separates transmural from transpapillary routes (20 codes, starting at 0F9480D). Other additions cover percutaneous division of native and prosthetic aortic and mitral valve leaflets (028F3Z0), percutaneous device insertion into the ventricular septum (02HM3JZ), extraction of omentum and mesentery, bladder-to-ureter transfer in the new 0TX table (0TXB0Z6), resection of the prostate with the capsule intact (0VT00ZE), and pleural and mediastinal drainage devices with active mechanical clearance (0W99003).

Section X: New Technology

This section received 68 of the 181 new procedure codes in the release, 38% of all additions. New codes include X0HQ05C (Insert Leadlss Neurostim Gen in Vagus Nrv, Open, New Tech 12), X2723CB (Dilate IVC n Confl w Opn Polmr Lattc, Perc, New Tech 11), and X27P3NC (Dilate R Ant Tib Art w Sm-diam Intlm Dev, Perc, New Tech 12). It also carries 2 of the 2 revised code descriptions.

Of the 68 Section X additions, 52 carry new technology group 12, the FY 2027 group (7th character C). The other 16 carry group 11 and were first released in the April 1, 2026 update, after the October 2025 code set. The group 12 codes center on cardiovascular devices, including small-diameter intraluminal devices for dilating the tibial and peroneal arteries (X27P3NC), transcatheter replacement of the aortic and mitral valves and open replacement of the pulmonary valve, and branched aortic restriction systems. They also add spinal stabilization devices at the cervical, cervicothoracic and thoracic joints (12 codes, starting at XRH10NC), single-use scopes for inspecting the hepatobiliary and pancreatic ducts (XFJB3CC), computer-aided detection and notification for CT, echocardiogram, electrocardiogram and vital-sign findings, and new drugs such as elamipretide (XW0134C), landiolol (XW0336C) and zimislecel (XW0337C). Section X also loses 38 codes, all from new technology groups 7 and 8 (FY 2022 and FY 2023). Some technologies now have permanent codes in other sections, such as the custom-made interbody fusion devices and high-dose intravenous immune globulin; the rest are drugs and devices such as terlipressin, trilaciclib, lurbinectedin, amivantamab and fostamatinib.

Section F: Physical Rehabilitation and Diagnostic Audiology

This section received 23 of the 181 new procedure codes in the release, 13% of all additions. New codes include F07DYD0 (Electrotherap, Integu Head, Neck, Micrcrnt Stim), F07EYD0 (Electrotherap, Integu Thorax/Abdomen, Micrcrnt Stim), and F07FYD0 (Electrotherap, Integu Up Back/UE, Micrcrnt Stim).

The rehabilitation additions extend wound management treatment to the thorax and abdomen for both the integumentary and musculoskeletal body systems, across every equipment value from negative pressure therapy to no equipment (18 codes, starting at F08E5AZ). Five more codes add microcurrent stimulation electrotherapy for the integumentary system of the head and neck, thorax and abdomen, upper back and upper extremity, lower back and lower extremity, and whole body (F07DYD0).

Section 3: Administration

This section received 6 of the 181 new procedure codes in the release, 3% of all additions. New codes include 30233S5 (Transfuse Hi-Dose Imm Globlin in Periph Vein, Perc), 30243S5 (Transfuse Hi-Dose Imm Globlin in Central Vein, Perc), and 3E00XGF (Introduce Oth Enzym/Chem Agt in Skin/Mucous Mem, Extern).

The Administration additions are high-dose immune globulin transfusion into a peripheral or central vein (30233S5, 30243S5), which replace the Section X codes XW133D7 and XW143D7, external introduction of other enzymes or chemical agents into skin and mucous membranes or subcutaneous tissue (3E00XGF, 3E01XGF), and introduction of embryonic stem cells into a peripheral or central vein (3E033AZ, 3E043AZ). The embryonic stem cell codes replace the transfusion codes 30233AZ and 30243AZ, the section's only two deletions.

Smaller sections worth knowing

Extracorporeal Assistance and Performance adds cardiac output assistance with a blood flow modulator (5A0221E), circulatory decompression with an impeller pump (5A0527D), and urinary filtration with selective cytopheresis in three duration bands (5A1D701). Imaging gets a new table, B85, for other imaging of the choroid and retinal vessels using a fluorescing agent (B85A2ZZ), and Measurement and Monitoring adds body composition by air displacement plethysmography (4A0FXEJ). Two existing Section X codes were retitled (XW0V0P7, XW143F8).
