Injury of heart (S26)

ICD-10 code S26 covers various types of injuries to the heart, including contusions, lacerations, and unspecified trauma, both with and without hemopericardium. These codes specify the nature and severity of the heart injury, the presence of blood in the pericardial sac (hemopericardium), and the stage of medical care such as initial encounter or sequela.

This section is used to classify detailed heart injuries, helping differentiate conditions like heart injury with hemopericardium; sometimes called traumatic hemopericardium or closed heart injury with hemopericardium; from those without blood accumulation around the heart. It also distinguishes injuries by severity, for example, mild, moderate, and major lacerations. The codes include specific seventh-character extensions to indicate initial treatment, follow-up visits, and complications or late effects. Synonyms like heart injury, closed, contusion to heart, or perforation of cardiac wall connect clinical terminology to the appropriate ICD-10 code, ensuring accurate use for medical coding and billing related to traumatic cardiac conditions.

Instructional Notations

Code Also

A "code also" note instructs that two codes may be required to fully describe a condition, but this note does not provide sequencing direction.

  • any associated:
  • open wound of thorax S21
  • traumatic hemopneumothorax S27.2
  • traumatic hemothorax S27.1
  • traumatic pneumothorax S27.0

7th Character Note

Certain ICD-10-CM categories have applicable 7th characters. The applicable 7th character is required for all codes within the category, or as the notes in the Tabular List instruct. The 7th character must always be the 7th character in the data field. If a code that requires a 7th character is not 6 characters, a placeholder X must be used to fill in the empty characters.

  • The appropriate 7th character is to be added to each code from category S26

7th Character

Indicates that a seventh character is to be assigned to codes in a subcategory.

  • A - initial encounter
  • D - subsequent encounter
  • S - sequela

Clinical Terms

The following clinical terms provide additional context, helping users better understand the clinical background and common associations for each diagnosis listed in this section. Including related terms alongside ICD-10-CM codes supports coders, billers, and healthcare professionals in improving accuracy, enhancing documentation, and facilitating research or patient education.

Pericardium

A conical fibro-serous sac surrounding the HEART and the roots of the great vessels (AORTA; VENAE CAVAE; PULMONARY ARTERY). Pericardium consists of two sacs: the outer fibrous pericardium and the inner serous pericardium. The latter consists of an outer parietal layer facing the fibrous pericardium, and an inner visceral layer (epicardium) resting next to the heart, and a pericardial cavity between these two layers.

Pneumopericardium

Presence of air or gas in the space between the heart and the PERICARDIUM. The degree of respiratory distress depends on the amount of trapped air and circulation blocked in the systemic and pulmonary veins.
